RELEASE OF LIABILITY FOR CAMP CAPTAIN PARTICIPANTS (Ages 13–18)
MUST READ AND AGREE BEFORE SIGNING
1. Acknowledgment of Risk and Fitness
I acknowledge that this Camp includes multi-sport events, physical activity, organized play, leadership responsibilities, and assisting younger campers, which carry the potential for serious injury, illness, property damage, or death. I certify the Camp Captain is physically fit to participate and has no condition that would endanger herself or others. I accept responsibility for conduct in connection with the Camp.
2. Assumption of Risk & Release
I acknowledge that participation is inherently dangerous. On behalf of myself and/or the Camp Captain, I WAIVE, RELEASE, and FOREVER DISCHARGE Greater Kansas City Sports Commission and/or Greater Kansas City Sports Foundation, affiliated organizations, facilities, sponsors, staff, volunteers, and related entities (collectively the “Released Parties”) from any and all claims arising out of participation in the Camp, including negligence, medical treatment decisions, property damage, or travel to and from the Camp.
3. Medical Procedures
If the Camp Captain becomes injured or ill, Camp staff will contact parent/guardian or emergency contacts and may contact emergency medical services if necessary. I authorize medical professionals to provide treatment including hospitalization, anesthesia, or surgery if required. I agree to assume all medical expenses incurred.
4. Assumption of All Risks
I acknowledge and ASSUME ALL RISKS associated with participation, including leadership duties and assisting younger participants. Participation is voluntary and I waive the right to sue for injuries or damages, including those resulting from negligence.
5. Covenant Not to Sue & Indemnification
I agree not to sue any Released Parties and agree to indemnify and hold harmless the Released Parties from claims arising from breach of this agreement or violations of Camp rules.
6. Media Release
I grant permission for WIN for KC and affiliated organizations to use the Camp Captain’s name, image, and likeness for promotional purposes in perpetuity without compensation.
7. Authority of Parent/Guardian (If Applicable)
If the participant is under 18, the signing parent/guardian certifies legal authority to bind the minor to this agreement and agrees to indemnify the Released Parties accordingly.
8. Registration Policies
Each participant must be registered individually. Registrations are non-transferable. Camp may be cancelled due to safety concerns.
9. Severability
If any provision of this agreement is deemed unenforceable, the remaining provisions shall remain valid.
